I really like this a lot. And I don't even mean the fact that it is framed nicely, is direct, and looks sharp. Though all those are true.
This is so quirky. It could mean a lot of different things, and yet it is the most fun if it means nothing. I feel like perhaps it was somebody's prank, and we're looking in closely at it like 'huh?' particularly given the title. Or, even it could be, you're in so much pain you're seeing brightly colored fruit, and thinking, hm, maybe Motrin is not going to do me any good right now. Or maybe your liver is failing from too much ibuprofen because you have been eating it like candy, and now you are hallucinating that it actually -is- candy. (Maybe it wouldn't work that way, actually, but it's still an interesting concept.)
Last but certainly not least, runts are fun. The lighting and close shot brings out the luster in them. The idea that runts candies themselves ease someone's pain isn't that far off. It at least makes you smile.
All I'd say in terms of constructive criticism is that putting it on a non-specific white surface gives it more the feeling of being an advertisement for a real product than a shot from life. It was a bit difficult for me to tell which you were going for.
